Objective To analyze the current situation of nursing needs and services for the aged in Sichuan province,so as to provide references for improving and standardizing nursing needs assessment and services for the aged. Methods The data of geriatric nursing needs assessment carried out by Sichuan Provincial Health Commission were used. The elderly aged 65 years and over were selected as the research subjects. The ability classification of the elderly, the number of geriatric syndromes, the level of nursing needs, whether to provide nursing services, and the items of nursing services were included in the descriptive analysis. Results A total of 1 272 896 elderly people were enrolled,of which 962 607(75.62%) were in 0 grade(good ability). The number of cases of geriatric syndromes were 0-2,which accounted for the majority. A total of 310 289 elderly people had different degrees of nursing needs, and 129 097 elderly people were provided with nursing services by nursing staff, accounting for 41.6%. The highest proportion of the 4 types of nursing services were safety protection and guidance, health education, psychological communication and counseling, and traditional Chinese medicine diet care. Conclusion The nursing needs level of the elderly in the community is mainly intact, and the number of geriatric syndrome projects is mostly 0~2. The supply of nursing services for the elderly with nursing needs is seriously insufficient, and the supply of nursing services for the elderly needs to be standardized and improved.

To summarize the nursing cooperation experience of a spinal cord injury patient with spinal cord nerve stimulator implantation in the treatment of paralysis of both lower limbs. Rigorous perioperative care was provided for patients;multidisciplinary collaborative diagnostic and treatment teams were established and participated in;a surgical support system was established;close observation and timely feedback on the operation of electrophysiological monitoring was conducted to ensure the accuracy of surgical operations;the safety of surgical posture was assessed using the CORN Intraoperative Acquired Stress Injury Risk Assessment Scale;fine cooperation with patients during the awake period of surgery was to ensure accurate electrode placement;stringent risk management and control of incision infection and sound defense mechanisms were conducted. The patient’s surgery was successful, and a month after the surgery, the electrodes were turned on,and sensation appeared in the hip and left lower limb.

Objective To investigate the effect of specialty nurse-led discharge preparation in the perioperative period of lung transplantation patients. Methods Based on the transition theory, literature analysis and expert meetings, the hospital discharge preparation service plan for lung transplant patients was established. We selected 33 lung transplant patients in a tertiary hospital in Zhejiang Province for this study based on convenience sampling, and they were divided into an experimental group and a control group by random number method. The experimental group received the discharge preparation service program led by specialty nurses, and the control group received routine perioperative management of lung transplantation. Data from admission to 3 months after discharge were collected. Between the 2 groups, they were compared on readiness for hospital discharge, reintubation, readmission, and part 2 of Partners at Care Transitions Measure(PACT-M2). Results A statistically significant difference was found in the scores for readiness for hospital discharge, reintubation rate, and Chinese version PACT-M2(P<0.05). There was no significant difference in the number of readmissions 1 month and 3 months after discharge between the 2 groups(P>0.05). Conclusion Specialized nurse-led discharge preparation services for lung transplant patients can improve their readiness for discharge and enhance the quality of self-care at home.

Objective To develop a physical restraint management system for ICU patients and explore its application effect. Methods A research and development team was established to build a physical restraint management system for ICU patients based on the ICU patient physical restraint assessment scale. The system collects relevant data in real time to realize the functions of dynamic reminder and quality control of patients’ physical restraint decision-making. A total of 216 patients admitted to the ICU of a tertiary A hospital in Zhejiang Province from January 2022 to June 2022 were selected as the research subjects. The patient’s physical restraint rate, length of physical restraint and the incidence of unplanned extubation of the system were compared after application(April to June 2022) and before application(January to March 2022). The nurses’ satisfaction with the management system was collected. Results After the application of the physical restraint management system for ICU patients, the physical restraint rate was 35.19%,which was significantly lower than 49.07% before application. After the application, the physical restraint time of the patients was 45.71(16.41,121.31) h,which was significantly shorter than 126.00(28.05,317.00) h of the control group. There was no significant difference in the incidence of unplanned extubation between 2 groups. The total average score of ICU nurses’ satisfaction with the system was (4.56±0.48) points. Conclusion The use of the physical restraint management system for ICU patients can help reduce the use rate and duration of physical restraint in patients, and nurses are more satisfied with the system.

Objective To construct an early enteral nutrition tolerance monitoring system for ICU patients and to apply it into the practice. Methods The enteral nutrition tolerance score table was revised, and on this basis, the early enteral nutrition tolerance monitoring system of ICU patients was constructed to collect relevant data in real time. Using the convenient sampling method,120 patients with early enteral nutrition treated in ICU of a tertiary A hospital in Zhejiang Province from March to August 2021 were selected as the research subjects. The monitoring system was used to evaluate the tolerance to enteral nutrition of the experimental group;the revised enteral nutrition tolerance rating scale was used to evaluate the control group;the enteral nutrition scheme was dynamically adjusted according to the evaluation results. The incidence of feeding intolerance within 7 days of enteral nutrition, the target feeding rate, and the difference of albumin, serum total protein and hemoglobin concentrations between the 8th day and the 1st day of enteral nutrition were compared between the 2 groups, and the nurses’ satisfaction with the monitoring system was collected. Results The incidence of feeding intolerance in the experimental group was lower than that in the control group;the target feeding rate was higher than that in the control group;the difference of albumin, serum total protein and hemoglobin concentrations between the 8th day and the 1st day of enteral nutrition was higher than that in the control group(P<0.05). The total average score of nurses’ satisfaction with the monitoring system was(4.51±0.31). Conclusion The use of early enteral nutrition tolerance monitoring system for ICU patients can effectively reduce the incidence of early enteral nutrition intolerance, improve the target feeding rate and improve the nutritional status of patients. Nurses are highly satisfied with the monitoring system.
